Welcome to the Northwest Technical College CARDIAC SONOGRAPHY application. This application is for students seeking admission to the FALL 2026 COHORT START. Please read all instructions carefully and ensure you meet the eligibility requirements before submitting your application.

Applicants will be required to upload digital copies of their HSRT-AD exam report and proof of CNA as part of this application. Please ensure you have legible electronic copies of these documents ready before you begin.

Important Notice: It is the applicant’s sole responsibility to upload clear and readable documents. Illegible, incomplete, or corrupted files will not be reviewed, and the Cardiac Sonography Admissions team is not responsible for interpreting, reconstructing, or verifying unclear submissions. Applications containing unreadable or missing documentation may be considered incomplete and therefore ineligible for review.

3. How are you meeting the CNA requirement?*

I have completed/am currently enrolled in HLTH 1110: Nursing Assistant at NTC or an equivalent course within the last 5 years

I have an active CNA certification or had one within the last 5 years

I have a CNA Waiver approved by the NTC Cardiac Sonography Faculty

I understand that providing false, misleading, or incomplete information may result in my application being rejected or not reviewed for admission to the Northwest Technical College Practical Nursing Program for the Fall 2026 start.
